I didn't have a problem with it since it was a for profit baseball academy and the people used their hard earned money to buy the stuff. I’m guessing that sheet is used at a tryout and each player is rated at 1 to 5 in every category for the aggregate of the tryout. Could you give an example of the evaluation for 1 player? End of season player evaluation form?
